Isn’t this just easy 100% uptime on Gluttony of Elements (and therefore become immune to damage?).
Gear: Brutal Restraint for The Traitor Keystone, 2 Soul Ripper with roll range, Foulborn Cloak of Flame, Blood of Corruption, Runegraft of the Warp
Passives need: Priest of Yaomac from Bloodline Ascendency, the Acendency node from Harbinger that gives 30% slower buff expiry.
Some untested mechanics:

Buff slower expiry rate can stack ADDITIVELY to 60% with Harbinger + Runegraft, effectively granting 1/0.4=2.5x Buff duration.

Buff slower expiry rate affects buff duration of Gluttony of Elements, but NOT soul gain prevention duration.
If those mechanics work, then some maths:

Buff Duration : 5 (base) * 1.1 (10%more Mastery) *2.5 (buff 60% slower) * 1.65 (65% increased duration from passives) = 22.7s

Duration of soul gain prevention: 14 (base) * 1.1 (10%more Mastery) *1.45 (65% increased duration AND 20% reduced from Bloodline) = 22.3s

Flask Charges gain in 20 sec (3 empty slot = 2.4c/s, 2 flasks): 2,4*2*20 = 96 charges. Gluttony of Elements need 70 Souls (30% less soul used from Bloodline node)

To become immune to physical: use Cloak of Flame and keep Fire resistance below -50% (can use Foulborn Cloak of Flame with no +fire res). 40% of damage taken from phys is taken as Fire and is amplified to 60% with negative fire res. This will be enough to heal against the original 60% phys damage. Cloak of Flame converts even phys dot (bleed and corrupted blood). For chaos damage just use CI.
Last thing to do is figure how to press the button as soon as the buff wears off, and figure how to do damage.
You can also min max duration and other factors such as flask charges gain or phys taken as other elements, but this is the simplest setup I can come up with.
