League started this build and it's quite impressive. Killed all content up to and including end game bosses, but not tested against "aspirational" content such as 100% Delirious T17s. Huge damage, poison proliferation, whirling blades, and near infinite flasks make for great quality of life.
Clear speed is largely provided by Bino's Kitchen Knife for Poison Proliferation, which is incredibly helpful in clearing and deleting single targets surrounded by enemies (such as Metamorphs). The damage isn't instant like HoI/Profane Bloom/Asenath's, so it might "seem" worse, but is actually very comparable in clearing power. Outside of this point, the weapon also provides great stat lines for the build in general as well as having an enormous base crit chance to ensure easy crit capping for our trigger skill.
Main setup is Blade Flurry - CoC - Reap. Less damage than cyclone-based setups but you're able to play like a ranged character and not get deleted by anti-melee garbage. Cyclone + fixing APS/CDR would be more DPS, but worse survivability in my eyes.
Since we are not going for the 10 APS breakpoint, we get to skip on Awakened CoC as well as CDR belt. It's worth noting as well that blade flurry accounts for a reasonably large portion of the total DPS compared to Cyclone. Enough to be non-negligible (especially for leech).
Awakened Spell Cascade along with Watcher's eye with Life Gain on Hit w/ Vitality and Flask Charge on Crit w/ Precision is key for making the life/mana sustain fit together.
Since we are playing CoC poison, we can apply wither with Ancestral Protector + Withering Touch, which is slightly better QoL than Wither + Spell Totem. This stacks up wither quite quickly on bosses and offers great body block utility.
This build is incredibly mobile, using Whirling Blades with high APS to zoom around as well as Withering Step + Flame Dash for all the utility they provide. Flame dash can be swapped to Frostblink if preferred, but Withering Step provides too much incidental value IMO to consider dropping.

Defensively (outside of life + capped ele resists, ofc), we run:
It is worth noting that a huge part of good defense on this build is getting a lot of hits in, which means good offense/uptime. This means that your defense/recovery starts falling apart if you use too many life flasks while running away and not attacking, for example.
For life sustain, we run a precision/vitality watcher's eye with life on hit + flask charge on crit. I also highly prioritize life regeneration on gear as a "bonus" stat (it's usually priced as a "worth nothing" modifier). The goal is to out-regen Blood Rage by a fair amount so that we can still be safe while not attacking. Blood rage also provides incidental life leech (though not particularly notable). It should be noted that sustain is considerably worse on single target encounters, but this is the reality of any life-on-hit build.
CWDT + Storm Brand/Blade Vortex play a key role in providing additional recovery while not attacking. Outside of this, life flasks are the only other way of keeping yourself alive while not attacking.
For mana sustain, the entire build can be sustained on an enduring mana flask pretty easily (piggy backing off the flask charge on crit setup), but I decided to make some additional investments to fit clarity@7 + a crafted flat life/mana regen modifier for any open crafted life prefix. This allows us to cast our utility at will without needing to leech. A mana leech ring helps push the mana regeneration further to sustain face tank situations where you will be casting a lot of Storm Brands/Blade Vortexes.
Sustain can be scaffolded by Clever Thief (easily reached by this build). However, those passive points will be needed eventually to push more DPS/survivability.
Note: I added Anomalous Blood Rage for true single target frenzy charges which I've been meaning to fit in for a while. I finally bit the bullet and bought it.
